CHS Girls Volleyball Summer Camp 2021 Interest List

Camp is not a requirement for trying out for one of the Chaparral teams. All players MUST have completed the Athletic Physical Process in order to attend the camp. 

In order to play volleyball, all athletes must be cleared through the School Athletic Department. THIS MUST BE COMPLETED BEFORE SUMMER  CAMP, TRYOUTS OR PRACTICING WITH THE TEAM.

The first step is to log into the clearance website and complete the athletic packet for your student athlete. The website you will go to is:  https://www.athleticclearance.com. Once completed print and sign the Athletic Clearance form (one form).

Your student athlete must also have a physical completed by your doctor. You will need to take this Physical Evaluation History and Examination form to your doctor and have them sign and stamp it. Per CIF rules, no student-athlete will be allowed to attend a camp or try out for a team without a completed physical form signed by a physician. NO EXCEPTIONS.​

Once completed return the physical form and signed clearance form to the Athletic Department to be cleared. The office is currently closed but we will notify you when the office is open. An email confirmation will be sent to you once your child has been cleared to play Volleyball.
